New Patient Registration
Our practice area comprises Halfway Cambuslang, Drumsagard Village, Blantyre, High Blantyre extending to Burnbank as far but not including Hillhouse in Hamilton.please click on practice boudary tab and type your poste code and search,if you are located within the yellow colour map you can register with us.
All new patients registering with us are invited to attend for a short medical check-up to ensure your records are up to date. To enable you to register with the practice you should bring your NHS medical card. You will be asked to complete a form and will be offered an appointment to attend the practice sister. Please bring with you any medication you are taking and a urine sample.
Please note if you fail to attend this appointment you will not be registered with us.
Online 'Pre-Registration' With The Practice
If you wish to register with our practice you must complete a registration form for each family member registering. Registration forms can be downloaded using the link below, or collected from our practice reception at any time. You should then visit or telephone the practice to arrange a brief medical assessment with our practice nurse, Sister Wilson. You should bring 2 forms of photographic identifications to confirm your identity, along with all of your currently prescribed medication and a urine sample (sample containers can be collected from our reception). Until you have attended for your registration medical, you will not be registered with the practice.
Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
To be able to open the form you need to right click on the following file and save it on your computer first.Then you can type and print the form or print and fill it. Please bring the signed form to the surgery for registration,and don't forget to bring the asked IDs.
In the current coronavirus pandemic we ask you to fill the form online and along with a copy of your photo ID to email it to practice please.
